Kent Valley Hockey Association Team Fundraising Reimbursement Form This form serves as official notification to the Kent Valley Hockey Association of the fundraising acitivity. When this form is filled out and submitted to the KVHA main office along with all collected/donated funds the team will receive a reimbursement check from the KVHA Treasurer. Reimbursement will be within 3 to 4 business days.
